STROKE SURVIVORS HAVE A HIGH RATE OF RECURRENT STROKE AND HOSPITAL READMISSION, YET FEW HAVE ACCESS TO REGULAR NEUROLOGICAL CARE.

OUR GOAL IS TO MAKE EXPERT, PERSONALIZED CARE ACCESSIBLE TO ALL STROKE SURVIVORS.

WHAT TO EXPECT

As part of the pilot, patients are sent a blood pressure monitoring device, a wrist wearable, and a smart pill box. Patients have the option to use their own wearable device (we integrate with Apple Watch, FitBit, and many other devices).

Patients are also given access to our app where they work with Syntrillo neurologists and rehab specialists who deliver personalized preventative and post-stroke care.

Please note: this study is restricted to participants at Valley Health in Virginia. Please contact your healthcare provider at a participating site for additional information.
