
Most stroke survivors have little continuity in expert neurological care after discharge despite a high incidence of recurrent stroke and hospital readmission.
Our experienced team of neurologists and stroke rehab specialists work closely with patients over the course of several months to monitor and manage stroke risk factors and unmet rehab needs.

have no planned outpatient Neurology follow-up
have hypertension and multiple vascular risk factors and may benefit from closer monitoring and continuity of care

PATIENT ELIGIBILITY:
Diagnosis of ischemic or hemorrhagic stroke in past 18 months
Is being discharged home or to independent living
If significant cognitive impairment, has active caregiver that can participate
Ability to engage in a telehealth program
Access to internet
Owns a laptop, tablet, or smart phone
Lives in Virginia*
